| FBI toxicology tests have determined that three Americans that died in the Dominican Republic this past spring while on vacation died of natural causes. 41-year-old Miranda Schaup-Werner of Whitehall Township, PA, died in May at the Grand Bahia Principe in La Romana. Officials have released video from inside the PES refinery in South Philadelphia just before it exploded on June 21. A ground-hugging cloud of hydrofluoric acid can be seen creeping along before catching fire. The U.S. Chemical Safety Board found that an elbow in a pipeline hadn't been replaced in decades and the thickness of that pipe had degraded well below replacement levels. | A 10-year-old girl was killed after being thrown from an amusement ride at a fair in South Jersey on Saturday evening, 10/12.
